Doggy Play Camp Agreement Form

To ensure the safety and health of all animals and staff we require all clients to read and comply with the following Rules and Regulations:

SHOTS: All dogs must have up-to-date vaccinations. Parents or their vet's must submit documentation that their dogs have current DHLPP, Rabies and Kennel Cough vaccinations. 

HEALTH: All dogs must be in good health.

Parents will need to certify that their dogs are in good health and have been free from any condition that could potentially jeopardize other guests. Dogs that have been ill with communicable conditions in the last 30 days will require a vet's certification of health to be admitted or readmitted.
 
If, at any time during care, a dog is noticed to have fleas or ticks, treatment will be applied and charged to the parent at the minimum rate of $10.00. All dogs will be examined for signs of fleas and ticks at check in and will not be admitted to day care if fleas or signs of fleas or ticks are noticed. This “spot check” is not a foolproof process; therefore, we highly recommend a flea and parasite preventative, excluding flea/tick collars. 

COLLARS: Required with name tags.
 
BEHAVIOR: All dogs must pass Behavior Assessment for enrollment in our program. All dogs must be non-aggressive and not food or toy protective.

Parents will need to certify that their dog(s) have not harmed or shown any aggressive or threatening behavior towards any person or any other dog(s). Please remember that your pet will be spending time with other pets and that the safety and health of all the pets is our main priority. Keep in mind, although play is supervised, your dog still might acquire an occasional nip or scratch.

AGE:  Puppies may begin before they receive their Rabies vaccine as long as they are less than 6 months of age.

ENROLLMENT:  All dogs must have a complete, up-to-date and approved enrollment form on file.  All forms must be signed by the parent before admission.

FEES: Fees are due and payable at the beginning of each day.  For Sleepovers, ½ of the total due is required at the beginning of the stay.  Cash, checks, or money orders are accepted.  Discounted fees are available only on pre-paid packages.  Pre-paid packages are not refundable but are transferable. Our program is flexible enough to accommodate reasonable special needs requests; however, appropriate fees will be applied. 

HOURS OF OPERATION:  Day Care:  Monday through Friday from 7:00 a.m. to 6:00 p.m. Unscheduled pick up is usually available with a 1 hour notice.  Weekend and holiday pick up times MUST be scheduled in advance. 

Doggy Play Camp Day Care closes promptly at 6:00 p.m. Monday through Friday. Late charges of $1.00 per minute will be applied after 6:00 p.m. If you are running very late, it may be more economical to call and change your dog's visit to an overnight stay. 

RESERVATIONS:  Suggested but not always required.  Please call for availability.

For myself, my heirs and any assigns, I hereby release Doggy Play Camp, its agents, officers, subcontractors, employees, animal parents, customers and potential customers of Doggy Play Camp from any and all liabilities for injuries to my dog, or any other property of mine which arise in any way out of services and/or products provided by or as a consequence of my association with Doggy Play Camp. I acknowledge and understand that every dog reacts differently and that animals, by nature are unpredictable.

Dogs and animals may, without warning, bite or cause injury to humans and other dogs. I acknowledge and understand that there are certain risks involved in participating in day care, overnight care, or receiving care in your home, including but not limited to fights between pets, dog bites to humans or other dogs, and the transmission of disease.

In the case of emergency or for the use of the Dog Transportation services, I recognize the risks of injury that accompany said transport and acknowledge that this RELEASE is being relied upon by Doggy Play Camp to permit transport of my pet to and from Doggy Play Camp or any other necessary location. Furthermore, I accept any and all conditions, rules and regulations promulgated by Doggy Play Camp associated with the activities.

Dogs not familiar with Doggy Play Camp may experience separation anxiety when apart from their human companions.

Dogs not regularly exposed to the level of activity at Doggy Play Camp may feel the discomfort of sore muscles, sore joints and fatigue.

Dogs not regularly socialized do not necessarily know how to behave politely with other dogs. These dogs are at higher risk of incidents including, but not limited to, bites, fights, fear aggression, object guarding, and behavior problems and/or acting out.

Dogs at play during the regular day care do get dirty.

I understand the extra risks my puppy has of contracting disease or illness by entering the day care program without being fully vaccinated.

Dramatic changes in food and/or food quality may cause upset stomachs, diarrhea, and/or colitis.
 
I agree to pay for all services due at the time they are rendered. I understand any unpaid fees by me will be sent to collections and I will be responsible for all collections and legal fees incurred by such actions taken.

I understand and agree that in admitting my dog(s) into Doggy Play Camp, that Doggy Play Camp has relied on my representation that my dog(s) have not harmed or shown aggressive or threatening behavior towards any person or any other dog.

I certify that my dog(s) is/are in good health and has/have not been ill with any communicable disease within the last 30 days.

I understand Doggy Play Camp staff gives all pets involved in any type of incident a cursory examination, however, Doggy Play Camp is not liable for the location, treatment or diagnosis of any injuries incurred on our premises. It is recommended you check your dog further or seek treatment for your pet by a licensed veterinarian at your discretion and cost.

I, grant Doggy Play Camp and/or its selected agents full power of decision concerning the care and well being of our dog(s). Should any medical emergency arise, it is agreed that Doggy Play Camp or its selected agents can and will make any needed decision concerning medical treatment.  With my signature below, I accept exclusive and sole responsibility for any medical treatment charges incurred and release Doggy Play Camp of all liability, no matter the cause.
